New official prescription forms have started being issued as of September 1, 2018. These new forms are for all practitioners that are authorized to prescribe schedule II controlled substances.
The new forms include advanced security features to address fraudulent forms being made. The new security features include a watermark pressed into the sheet that can be seen from either side of the paper. This feature prevents any potential duplication by scanning or copying. On the front of the new forms will also contain an “Rx” symbol in thermodynamic ink. When rubbed, the symbol will temporarily fade and then reappear again.
Any triplicate forms or prescription forms issued by the Department of Public Safety prior to September 1, 2018 will be considered invalid. Effective June 1, 2019, only the new official prescription forms will be accepted. To prepare for the new change, give yourself plenty of time to make arrangements to order the new forms because due to high demand they are requesting 6-8 weeks’ time to process orders.
